Cortisol Symptoms and Adrenal Fatigue: What Testing Can Prove

Adrenal fatigue is not a recognised diagnosis. No endocrinology society has accepted it, and a review of 58 studies on cortisol and tiredness found conflicting results almost every time 1. Why your doctor says adrenal fatigue does not exist

Some doctors and media sources still use the term 1.

A systematic review, which gathers every study it can find on one question, looked at 58 studies on cortisol and tiredness. Thirty-three were in healthy people and 25 in people with symptoms 1. The results conflicted almost every time, whatever test was used and however well the study was run 1.

If tired people shared one cortisol signature, those studies would line up. They did not, and no coherent pattern emerged 1. That is the gap your doctor is pointing at when the label comes up.

What saliva, blood, urine and hair cortisol each measure

Cortisol is the main stress hormone. Each sample type catches a different slice of it.

  • Saliva on waking and through the morning: the most used fatigue tests in this research. They measured cortisol right on waking, the rise over the hour after waking, and the pattern across the day 1.
  • 24-hour urine: the total cortisol passed over a full day 2.
  • Hair: cortisol built up over a longer stretch 2.
  • Blood: in burnout, blood cortisol did not differ from healthy people 3.

Two further tests give the adrenal glands a push or a brake. In the ACTH test, you are given the hormone that tells the glands to release cortisol, and the response is measured. In the dexamethasone test, you are given a steroid that should switch your own cortisol down, and the drop is measured 2.

What a measurable cortisol problem looks like in ME/CFS and burnout

The clearest example is ME/CFS, a diagnosed illness with crashes after effort and slowed thinking. A review of 46 studies compared people with ME/CFS and healthy people of similar age 2.

Saliva cortisol was lower on waking and through the morning. Cortisol in 24-hour urine and in hair was lower as well 2.

The glands also released less cortisol when given the ACTH push. The dexamethasone brake switched cortisol down further than expected 2. The authors suggest this lower output may sit behind unrefreshing sleep, crashes after exertion, slowed thinking and less resilience to stress 2.

These results come from people who already had the diagnosis. They do not carry over to unexplained tiredness 2.

Burnout

The two burnout reviews disagree. The larger one, 31 studies, found no difference after dexamethasone 3. The smaller one, 16 articles, found exaggerated suppression, meaning cortisol dropped further after the steroid than expected, a lower-output pattern unlike the over-active one seen in depression 4. On the morning rise in cortisol and on blood cortisol, the larger review found nothing, and across 38 stress markers cortisol showed no consistent pattern 3.

What to do if you suspect a failing adrenal gland

Adrenal insufficiency means the glands cannot make enough cortisol. The research behind this page does not cover it, so this page gives no thresholds and does not say which test fits.

Fainting, collapse or severe weakness need same-day care. Do not wait for a test slot.

For anything milder, ask your doctor which test fits your symptoms, and why that one.

What to check when cortisol is not the answer

Your tiredness is still there after a normal cortisol result, and other causes are worth testing.
