Which Hormone Tests Rule Out a Hormonal Cause of Weight Gain?

Only a few blood tests can find a hormonal cause of unexplained weight gain, and the list is short: thyroid and blood sugar for almost everyone. The fork is one question: do you have a specific sign, such as irregular periods or purple stretch marks? No sign, those two tests. A sign, one more branch, and your doctor decides it.

Ask for these two tests first

At your next appointment, say: "I have gained weight I cannot explain. Could I have my TSH and free T4 checked, and an HbA1c?" TSH is the signal your brain sends to the thyroid. Free T4 is the thyroid hormone itself, the part that is loose in the blood and free to work. HbA1c is your average blood sugar over recent months.

When the results come back, write down each result, its date, and the lab's lower and upper limits, the small numbers printed beside yours.

Also write down the month your weight started to move.

What a normal thyroid result closes, and what it leaves open

The thyroid sets how fast the body burns energy. When it slows, the brain raises TSH to push it. That is why TSH goes first: it shows trouble before the thyroid hormone itself falls. Free T4 sits beside it, so the doctor sees both the signal and the response.

A normal pair usually closes one door: a thyroid running too slow or too fast as the reason for your weight.

One point is open. Doctors disagree on whether to test thyroid antibodies, the immune markers that show the body attacking its own thyroid, when TSH is normal. I have no study that settles it. Ask your doctor which side they take.

Blood sugar is on the short list for a plain mechanism. Sugar sticks to red blood cells, and those cells live for months, so HbA1c reads as an average rather than a single moment. Insulin is the hormone that moves sugar out of the blood and into storage, and trouble handling sugar is tied to fat gathering around the middle.

Which signs add a test to the list

Irregular or missing periods, extra hair on the face or body, and stubborn acne point towards androgens, the male-type sex hormones, and towards polycystic ovary syndrome, PCOS, a condition only a doctor can diagnose. The blood panel for it is sex hormones taken at a set point in the cycle. The doctor chooses which ones.

Purple stretch marks, bruises that appear from nothing, weak thighs or shoulders and a rounder face point towards too much cortisol, the stress hormone.

Cycle changes, night sweats and broken sleep around the forties point towards perimenopause, the years when ovaries wind down but periods still arrive. Weight settling at the waist is part of that picture.

None of these signs is a diagnosis. Each is a reason to say the sign out loud, with the date it began. How many signs, or how strong, should trigger more testing is your doctor's call.

Why one cortisol or one estradiol result tells you little

Cortisol rises and falls through the day, peaking soon after waking. A single blood draw catches one moment on that curve. It cannot answer the question doctors ask when they suspect too much cortisol: does the body keep making it when it should have stopped? That question has special tests, set by a doctor. They are used when the signs above are present, not because weight will not shift.

Estradiol, the main oestrogen, and FSH, the brain's signal to the ovaries, swing in perimenopause. They can be high one week and low the next. One normal result does not exclude the change. One odd result does not confirm it.

Paid panels such as saliva kits, home kits, a "hormone age", leptin or a lone DHEA return figures, but no accepted way exists to connect those figures to weight gain. Neither you nor the person selling them has anything to read them against.

Bring a dated list of medicines and sleep changes

Some medicines can change weight. This is a well-known reason. Hormonal pills, antidepressants, steroid creams and inhalers all count. Do not stop or change any of them on your own. Bring a dated list: the month your weight began to move, and every medicine started, stopped or switched near it.

Put sleep on the same page. Broken sleep and weight gain often arrive together, and which pushes the other is hard to untangle. Write down how your nights changed and when.

What the word "normal" on your printout answers

Look at the printout again. Beside your result sits the word "normal". The lab's software printed it the moment your number landed inside its range. No one weighed that word against your waist, your sleep or your age.

It answers one narrow question: is this particular hormone out of line. Your weight gain was never part of that question. It is still there, still real, and still waiting for a second one.

Honestly: what is settled and what is not

Doctors disagree on whether to test thyroid antibodies when TSH is normal, and I have no study that settles it. This page also cites no studies by name, so treat the test list as a way to prepare questions for your doctor, not as a diagnosis.

Questions people also ask

Does a normal TSH mean my thyroid is not the problem?

A normal TSH with a normal free T4 usually closes the most common thyroid explanation, an underworking or overworking gland. The question of further thyroid testing stays open, and you can ask your doctor about it.

Should I buy a hormone panel online to find out why I am gaining weight?

A panel without a sign behind it returns numbers but does not find a cause of weight gain, and nothing accepted exists to read those numbers against your weight. Start with TSH, free T4 and HbA1c through your doctor, and name any specific signs.

Can one estradiol or FSH result tell me if I am in perimenopause?

No. These hormones swing from day to day in perimenopause, so one result neither confirms nor excludes it. The result becomes useful when paired with the day of your cycle, and the decision belongs to your doctor.
