Ferritin Normal but Iron Low? Check Saturation and CRP

Neither number decides alone. Ferritin, the iron your body keeps in storage, can climb with inflammation while the iron travelling in your blood falls. Ferritin under 100, transferrin saturation (TSAT, the share of iron carriers loaded with iron) under 20% and a high C-reactive protein (CRP, a blood marker of inflammation): this page is for you 1. All three normal: look elsewhere.

Ask for three results from one blood draw

At your next appointment, say: "Could my ferritin be read together with transferrin saturation and CRP, taken at the same time?" Most printouts carry ferritin alone. Saturation and CRP show whether the ferritin can be trusted.

Write down each value, its date, and the lab's lower limit, the small number printed next to yours. Labs draw the line in different places, and nobody will note it for you.

If your printout has a serum iron figure but no saturation percentage, ask for the percentage. I found no cut-off for serum iron on its own, so I cannot tell you whether yours is low. Saturation is the number the guidance is built on.

If your results show ferritin in range, saturation low and CRP raised, say so in those words. The reviews describe that pattern as one that can sit alongside true iron deficiency 2. Whether it applies to you is for the person reading your results.

Labs and countries draw these lines in different places, so your printout may use other cut-offs. Bring the lab's own limits with you.

What each number on your printout measures

Ferritin is the stock: iron stored inside cells, in reserve. Serum iron is what is moving through your blood at the moment of the draw. TSAT, transferrin saturation, is the share of the iron-carrying proteins that actually have iron on board. CRP is a marker your liver releases when the body is inflamed.

Ferritin asks how much is in the cupboard. Serum iron asks how much is on the table.

Saturation is the more useful of the two circulating numbers, because the guidance gives a line for it: under 20% 1. That is why the first step above asks for saturation.

Why ferritin and serum iron move in opposite directions

Both numbers can be correct. Inflammation changes where the iron goes.

When the body is inflamed, macrophages, the immune cells that recycle old red blood cells, hold on to iron instead of releasing it. Stored iron goes up, so ferritin rises. Circulating iron goes down, so saturation falls 2.

This was measured in 238 children aged 6 to 72 months. Where CRP was raised, ferritin rose and serum iron fell 5. It is a children's study, and I use it to show the direction of the effect, not to describe your numbers. The adult description of the same mechanism comes from a 2025 review 2.

The same review says this pattern often coexists with true iron deficiency 2. A comfortable ferritin therefore says little about the iron reaching your tissues. Chronic inflammation is also listed as a risk factor for iron deficiency 1.

What gets missed when only ferritin is read

In 96 people with obesity waiting for weight-loss surgery, ferritin under 30 found iron deficiency in 3 of 10. Counting saturation and CRP as well found it in 7 of 10 3. The rest had ferritin between 30 and 100, a raised CRP of 5 mg/L or more, and saturation under 20% 3. This was one hospital, looking back at records, so it is a strong signal for that group and not a number for everyone.

In 125 pregnant women, iron deficiency by 36 weeks was found in half of those with obesity and about three in ten of those at normal weight, despite iron supplements 6. Pregnancy changes iron needs, so this tells you about pregnancy.

Why did your appointment end at one number? All 29 of 29 guidelines in a 2015 review use ferritin to define iron deficiency. About half also use TSAT 7. The ferritin cut-offs themselves run from under 30 to under 100 7. The system defaults to the one number every guideline shares.

Where ferritin and saturation stop being reliable

Reviews say plainly that ferritin and saturation are neither sensitive nor specific once inflammation is present, including mild inflammation you cannot feel 4. Two other tests do better: sTfR, a protein that rises when cells are short of iron, and hepcidin, the hormone that locks iron inside cells. Neither is offered everywhere, and results differ between labs 4.

I found no study that tested a non-pregnant woman with normal ferritin and low iron, and none that says how far to trust saturation once inflammation is present. What exists is pregnant women 6, surgical patients 3, children 5 and the next group.

Among 2,050 people with heart failure, ferritin did not separate who did worse. Low saturation and low serum iron did 8. In that group, CRP and saturation moved in opposite directions 8. They are much older and sicker than most readers of this page.

What carries over is the method: three numbers from one draw, read together. That is what the studies behind this page used.

Questions people also ask

Can ferritin be normal while serum iron is low?

Yes. In inflammation, macrophages hold iron inside, so stored iron and ferritin rise while circulating iron falls 2. In 238 children with raised CRP, ferritin went up as serum iron went down 5. Whether this applies to you is for your clinician to decide.

Which ferritin threshold applies if my CRP is high?

A 2025 review uses ferritin under 100 as the line for iron deficiency when inflammation is present, against under 45 without it 1. Guidelines vary, from under 30 to under 100 7. Ask which line your lab used.

Can a normal ferritin rule out iron deficiency?

Not on its own. In 96 people with obesity before bariatric surgery, ferritin alone found deficiency in 3 of 10, while adding saturation and CRP found it in 7 of 10 3. That group is specific, so take it as a reason to ask about your own result.
