Low Ferritin, Low Iron Saturation: Is It Iron Deficiency?
Yes. Low ferritin, the iron your body keeps in storage, together with low transferrin saturation, how full your iron-carrying protein is, fits iron deficiency 1. The fork: ferritin under 15 and saturation under 10% is the strictest category 2. Everything else depends on which line your doctor uses. The pair is a stricter filter: it counts fewer women, not sicker ones.

- Find ferritin, serum iron and TIBC (how much iron the carrier protein can hold) on the printout. Write the unit next to each.
- If a saturation (TSAT) line is printed, copy it. If not, work it out: iron ÷ TIBC × 100. Both must use the same unit, both µmol/L or both µg/dL.
- Write both numbers and the test date on one line.
- Put ferritin (ng/mL is the same as µg/L) against 15 [[38848068]], 25 [[38848068]], 30 [[41033542]] and 45 [[41033542]]. Read your grade below.

Why the same blood test gets a different verdict
One study ran 40,381 women aged 25 to 54 through three definitions of iron deficiency 2. Same women, same blood. Only the line moved.
- Ferritin under 15 and saturation under 10% together: about 1 woman in 22 (4.46%) 2.
- Ferritin under 15 alone, the WHO line: about 1 in 10 (10.57%) 2.
- Ferritin under 25: about 1 in 5 (21.23%) 2.
Those three groups are three different filters, not mild, medium and severe. A woman with ferritin 20 and saturation 14% falls outside the strictest definition and inside the line at 25 2 and the consensus line at 30 3. Demanding both numbers low narrows the group. It sharpens nothing else.
Doctors do not share one rulebook either. A review of 29 professional guidelines found that every one uses ferritin to define iron deficiency 4. Saturation appears as a second option in only 10 of the 22 guidelines the review checked, 45% 4. The thresholds themselves varied widely 4. Expert consensus proposes 30 for ferritin 3; the WHO uses 15 2.
Your doctor may have used any of those lines when he said "normal". The more low numbers a definition demands, the fewer women it counts.
A normal haemoglobin does not close the question
Iron deficiency exists without anaemia 1. Anaemia means low haemoglobin, the oxygen-carrying protein in red blood cells. The haemoglobin cut-off can miss women who menstruate 1.
The symptoms listed for iron deficiency are fatigue, weakness, trouble with exercise, poor concentration, irritability and low mood 1.
A normal blood count means the haemoglobin was normal. It does not mean the stores were looked at.

What to say when they call it normal
Say this: "Which ferritin line did you use to call this normal? Please write it next to my result."
Then: "I have my transferrin saturation too. Where does the pair fall?"
A ferritin of 32 clears the lines at 15 and 25 2 and 30 3, and sits under the 45 that one laboratory proposed 3. A "normal" can be correct against one line and wrong against another. Naming the line turns an adjective into something written down.

No study shows which pair of numbers feels worse
I could not find a study that compares women with one low pair of numbers against women with a slightly different pair. Whether one feels worse, or needs different handling, has not been measured. The 2024 numbers count how many women fall under each line 2. They say nothing about who is sicker.
The proposal to move the ferritin line up to 45 for women comes from one hospital laboratory, using its own patients 3. Its authors argue that healthy-looking women with low stores have dragged ordinary reference ranges downward 3. No other laboratory has repeated it. It is a proposal, not a standard.

If it does not go to plan
- The printout shows no saturation and no TIBCRead ferritin against the lines alone. Ask for saturation to be added to the same draw, or to the next one.
- The units on iron and TIBC do not matchDo not divide them as printed. Check whether one is µmol/L and the other µg/dL, or ask the lab for the saturation figure.
- The printed saturation differs from the one you calculatedCheck the units first, then ask the lab which figure it stands behind. Write down both.

Honestly: what is settled and what is not
The numbers on how many women fall under each line come from a volunteer screening study, not from patients followed over time [[38848068]]. The guideline review dates from 2015 and shows disagreement without explaining it [[26561626]].
What to do with this
- Find your ferritin and saturation on the printout and write both with the date.
- Ask your doctor which ferritin line was used and have it written next to the result.
- Check that iron, TIBC and saturation use matching units before you trust a calculation.
- Tell your doctor about recent iron tablets or infection.
- Bring your symptoms, not only the numbers, to the appointment.
